Social Prescribing Link Workers
Everyday life can be challenging, issues such as poor housing, debt, stress, isolation, addiction and bereavement can have an affect your physical and mental health and you may wish to seek additional support to help you work through a difficult time.
A social prescribing link worker is a member of the team who’s had specialist training that enables them to support patients in making positive changes in their lives, including:
- Supporting patients with long-term conditions, including self-management
- Creating personalised care and support plans, based on what matters to the patient
- Introducing patients to community groups
- Safeguarding vulnerable patients
Social prescribing is a non-medical way of making connections with services who can help and support you, either short or long term, helping you to make the changes you want to make to improve certain aspects of your life. These non-medical referrals can work alongside existing medical treatments with the aim of helping you to make some changes that will be sustainable to help improve areas of your life, helping you to stay physically and mentally well.
We offer a Social Prescribing Service at Abbey House Medical Practice and if you feel that this service may be beneficial to you please arrange telephone call consultation with the Social Prescribing Link Worker who will be happy to assist you.
